Looks like a rope ladder step. Helps climb up into the truck bed. Barnyards truck is SO TALL!
Do you hang the small loop on a cabinet door handle so you can step on the pipe and reach the top shelf?

Edit: It is a vertically challenged assistant! :surrender:

It's a threaded pipe with a rope going through it, tied together and looped on one end.

One guess has been pretty close. Remember this was made and is used by an old man with two artificial hips and a bad back, though being lazy he has used one long before any of those problems.
